Social Worker
We are looking for an enthusiastic and caring individual to join our Complex Care Program!
The Complex Care Program is looking for a patient-centered social worker to join our team. The right candidate for this position must be able to work in a fast paced, high volume environment. The person will work collaboratively with multiple team members to seek out, organize, and coordinate services for patients and their families within the Complex Care Network. The person in this position may be required to travel to neighboring counties. This person will have to opportunity to help and be a participant in a new initiative at Van Buren County Hospital and Clinics. A bachelor's Degree in Social Work or Human Services is required, license preferred.
